Health check appointment cancellation form

Cancellation of your booking can only be made within a ‘cooling off period’ of 14 days from when you make a booking.  Within this time, you are entitled to cancel and receive a full refund of the treatment fee in respect of services not received.  This is in accordance with the law when you (as a consumer) purchase the My Wellbeing Services online or over the phone.  Should you wish to cancel your booking, within the cooling off period, you should call the booking team on 0161 464 3009 or complete the form below.

Where the appointment you book means that you will receive the My Wellbeing First Service within your cooling off period, you acknowledge and agree that you will not be able to benefit from the full 14 day cooling off period.

Should you wish to rearrange the date or time of your booking you should call the booking team on 0161 464 3009 or complete the form below and one of our team will contact you to reschedule your appointment.  Changes to your booking can be made within 14 days of your My Wellbeing First Service appointment.
